IN ORDER TO ACCESS THIS REGISTRATION, YOU MUST HAVE A COMPLETED MEMBERSHIP APPLICATION ON FILE. PLEASE BE PREPARED TO COMPLETE THE CAMP REGISTRATION IN ONE SITTING AS YOUR PROGRESS WILL NOT BE SAVED.
- Full IEP stating ASD as area of eligibility
- Recent photo of your child
- Behavior plan (if using)
- Immunization record
- Insurance card
- Full Payment (scholarships and payment plans available - contact Garrett's Place to arrange)
